Holiday Vaccinations and Travel Clinic
The Surgery offer a full travel travel immunisation service, including anti-malarial's
and yellow fever
vaccination. This service is available to both our own patients and other
practices' patients throughout the Portsmouth/Hampshire area. Details of
immunisations that are recommended for specific countries can be found
here.
For more exotic foreign travel you may require additional immunisations.
What Immunisations/ Malaria prophylaxis might I need?
Details of your holiday plans can be handed in to the receptionist, or
alternatively complete this form and
submit your travel details to us.
The practice nurse deals with holiday vaccinations/malaria tablets etc, and
can advise once we have this information to hand. To ensure all vaccinations are
effective prior to departure we need to know the full details of your
travel plans at least 4
weeks prior to travelling (you may be travelling without cover if any
later!)
Many vaccinations are available on the NHS but some are on a Private basis, see
private charges for
details.
Health Insurance
Please don't forget travel insurance, for those travelling within the EEC, an
E111 form is now required for all travellers including children. The E111
system has been revised by the Government and is now a card based
system with all travellers holding their own 'credit card' sized identification.
Please note the old paper based forms are no longer valid.
